NCC ATHLETICS|GO NAVS

All Grade 9 Student-Athletes are required to complete the following forms before commencing in any NCC Athletics practices or competitions. Once all the forms have been submitted, the Student-Athlete will be notified of his/her clearance to participate in team activities.

 

Physical Information:

 

• Athletes who suffer from a medical condition may be requested to obtain a Physical from a certified doctor before commencing team activities.

• Please complete all the information on the following forms accurately to avoid a delay in your clearance.
